I live in the Philippines part of the year. And I have found that most upper class Filipinos in spite of driving miles past incredible poverty every single day of their life, in spite of employing lots of people from the overwhelmingly poor majority - they just live in an unreality that assumes "every body" lives just like them in gated communities and ostentatious wealth with children all going to elite private schools.
And they believe this illusion in spite of being confronted with reality on all sides every single day of their lives.
When John McCain posited that 5 million dollars per year is what defined someone as rich. I think that he genuinely believes that. Just as John McCain genuinely thinks undocumented Mexican farm workers make $50 per hour and average working Americans must make double that.
