That is what happened here. PCIntern was entirely within her professional bounds to respond as she did. If a patient doesn't want to hear her/his views, the patient shouldn't have brought it up.

Your suggestions sound perilously close to the behavior of bad boyfriends who order around waitresses because they think they are paying for the privilege and/or are recognized as such superior beings that the waitress gladly submits.
