I think you're right that things like bone structure matter. Also the general shape of your body/how big your breasts are - stuff like that. For example, for someone with, say, C cup breasts, being a size 2 or 4 at any height would probably indicate unhealthfulness of some kind.
For me, though, I would probably feel fat if I starting fitting into a size 10, given my frame and all of that... which isn't to say that any woman of my height who's a size 10 is fat.
I was just trying to make the point that there's nothing necessarily unhealthy about size 2 or 4 (which I do think was implied by a few other posts--not yours I don't think and certainly not the OP), and I do realize that the same certainly goes for sizes 10 or higher.
