The point is to see if lice are present, not to pick them out and go nummy nummy. From the footage it actually seemed like the doc was checking for head injuries, contrary to journalistic portrayal; typically you examine behind the ears and the nape of the neck for lice, preferably with a fine tooth comb. Maybe that's too gross for TV.
